 Certified True Copy (CTC) of Birth Certificate (BC) issued by the National Statistics Office (NSO) or CTC of BC issued by the Local Civil
Registrar duly authenticated by NSO.
2.
 NBI Clearance for Passport Purposes.
3.
 Baptismal Certificate or Voter’s Affidavit or Certified True Copy of Voter’s affidavit from COMELEC or any public document indicating date and
place of birth and citizenship.
4.
 1 Valid I.D.
 
If married :
 
Certified True Copy (CTC) of Marriage Contract (MC) issued by the National Statistics Office (NSO) or CTC of MC issued by the Local Civil Registrar duly authenticated by NSO.
